CHARACTER DEVELOPMENT

Organization: Child Advocates of Silicon Valley
Mission: Child Advocates mission is to provide stability and hope to abused and neglected children by being a powerful voice in their lives.
Focus Area: Character Development
Program Name: Developmental Assets
Program Impact: 160+ youth, primarily 11-16 years old
Program Description:The Sharks Foundation’s grant will help Child Advocates of Silicon Valley to incorporate the “developmental assets” model into their work to help foster children in character development and to prevent high-risk behavior.

Organization: Mid-Pen Resident Services Corporation
Mission: Mid-Peninsula’s mission is to provide safe, affordable shelter of high-quality to those in need; to establish stability and opportunity in the lives of residents; and to foster communities to allow people from all ethnic, social and economic backgrounds to live in dignity, harmony and mutual respect.
Focus Area: Character Development
Program Name: Youth in Action Leadership Development Program
Program Impact: 250 unduplicated youth in Santa Clara County
Program Description: The Sharks Foundation’s grant will help 250 at-risk youth living in low-income housing communities stay out of gangs/violence, develop healthy relationships, play positive roles in their communities, and build a wide array of skills and competencies.

EDUCATION

Organization: Boys & Girls Club of Silicon Valley
Mission: The Boys & Girls Club’s mission is to enable all young people, especially those who need them most, to reach their full potential as productive, caring, responsible adults.             
Program Name: Education Enrichment Program – After School Kidz Science
Program Impact: 900 youth ages 6-18 years old.
Program Description: The Sharks Foundation’s grant will purchase the After School Kidz Science curriculum which includes both science and math components. Having a more in-depth curriculum will encourage more children ages 6-18 to think and apply elements they have learned to expand their critical thinking and academic skills.

Organization: Bring Me a Book Foundation
Mission:Bring Me a Book provides easy access to the best children’s books and inspires reading aloud daily to children. It is Bring Me a Book’s vision to have a world where all children read and succeed.
Program Name:San Jose Elementary School Library Initiative
Program Impact: 4,000 students and 900 parents/caregivers
Program Description:The Sharks Foundation’s grant will provide books and literacy resources to a total of 41 elementary schools in San Jose. The initiative will provide approximately 4,000 students with new books for their existing Bring Me a Book bookcase libraries and 900 parents and caregivers with a First Teachers literacy training workshop.

Organization: City Year
Mission: City Year’s mission is to build democracy through citizen service, civic leadership and social entrepreneurship. City Year unites young people from diverse backgrounds for a year of full-time service, giving them the skills and opportunities to change the world. As tutors, mentors and role models, these diverse young leaders help children stay in school and on track, and transform the schools and communities around them.
Program Name: Whole School, Whole Child Program
Program Impact: 2,700 students
Program Description: The Sharks Foundation’s grant will help impact 2,700 San Jose students at-risk of dropping out. Corps members provide critical interventions both during school and after school, including one-to-one literacy and math tutoring, behavior and attendance coaching, parent engagement programming and a variety of fun enrichment activities. The overall objective is to involve, connect and engage the “whole school” and its support systems to facilitate the “whole child.”

Organization: Downtown College Prep
Mission: Downtown College Prep prepares underachieving students—who will be the first in their families to go to college—to thrive at four-year universities.
Focus Area: Education
Program Name: Sharks Reading Tanks
Program Impact: 730 students, grades 6-12
Program Description: The Sharks Foundation’s grant will fund 15 mini-libraries (“Sharks Reading Tanks”) in its three current school sites to provide books to students, helping get them to grade-level literacy by the end of the 10th grade.

Organization: Sacred Heart Nativity School
Mission: Sacred Heart Nativity School is dedicated to providing quality, low-cost private education to at-risk youth students in the 6th-8th grades who are in danger of dropping out of school and risking a lifetime of poor outcomes.

Program Name: Nativity Middle Schools Classroom Instruction
Program Impact: 132 students in middle school
Program Description: The Sharks Foundation’s grant will purchase digital projectors and screens, and a color copier workstation for the students of the school. SHNS provides an academically challenging and highly structured environment with these unique characteristics: 1.) 44 more school days a year, 2.) 1:8 student to faculty ratio, 3.) 10.5 hour school day, 4.) Saturday school and summer camp, 5.) long-term relationships and support.

Organization: The Role Model Program
Mission: The Role Model Program’s culturally diverse, positive adult role models will inspire, motivate and empower underserved elementary and middle school youth to realize academic achievement, set ambitious dreams for their future, and prepare for post secondary education.

Program Name: The Community Classroom Visit Program
Program Impact: 3,800 youth
Program Description: The Sharks Foundation’s grant will partially fund the college preparation and career access program for 3,800 low-income, minority youth.

Organization
: Loaves & Fishes Family Kitchen
Mission: Loaves & Fishes Family Kitchen is committed to providing nutritious meals and support services in a safe, dignified and caring environment. They provide these services to all needy individuals with a special concern for families with children.
Focus Area: Health & Safety
Program Name: Food and Nutrition Program
Program Impact: 12,000 children
Program Description: The Sharks Foundation’s grant will help provide 12,000 children with meals through the Food and Nutrition program in 2011.

Organization: Pathways Hospice Foundation
Mission: Pathway’s mission is to provide compassionate, high-quality, family-centered home health and hospice care, promoting comfort, independence, and dignity. Pathways Hospice Foundation is the philanthropic branch of the Pathways Family of Services.
Focus Area: Health & Safety
Program Name: Pathways KIDS Program
Program Impact: 36 seriously ill and dying children
Program Description: The Sharks Foundation’s grant will provide health care and other visits in home for children with life threatening illnesses. The focus is on enhancing the quality of life for the child and family through the medical expertise of the Pathways KIDS team and its supportive care. The team is on-call 24 hours a day for consultation or home visits when needed.

Organization: Via Services
Mission: Via Services mission is to help children and adults with disabilities such as cerebral palsy, mental retardation, Down Syndrome, autism and other special needs.
Focus Area: Health & Safety
Program Name: Grow Smart
Program Impact: 100 youth with disabilities
Program Description: The Sharks Foundation’s grant will help the Grow Smart program address serious health issues (such as diabetes, vascular disease and osteoporosis) through educating 100 children with developmental disabilities about the importance of long term physical fitness and a healthy diet.
